- Detailed Course Outline
- The number of hours of instruction (theory and/or practical)
- Cost of the program
- Resume of the instructor
- Please indicate whether you would like to forward your course to the followng committees when the course is approved:
- advertising
- website
- newsletter committees
Course/activity givers must also provide anyone who participates in the course with a document that details:
- name of course,
- duration of course( number of hours),
- instructor's name and titles,
- participant's name, and the
- instructor's signature.
